The Hospice team provides end of life care to 400 patients per day in a wide variety of settings including homes, Adult Family Homes, Nursing Homes, Hospice House and Assisted Livings. We provide patient centered care focusing on comfort and quality of life. Staff are organized into multiple small teams based on geography and whether patients live in facilities or homes. Franciscan Hospice reaches patients in the 4,000 square mile area including King, Pierce and Kitsap Counties. Staff need to be willing to travel within their assigned area.

Role :

This job is responsible for overseeing and evaluating the day-to-day operations of assigned CHI Franciscan Health (CHI-FH) palliative care facility to optimize quality of care and support effective utilization of resources and achievement of desired clinical and financial outcomes. An incumbent promotes collaborative practice and continuity of care and is also responsible for ensuring that all work activities are compliant with CHI-FH policies and standards, palliative care clinical guidelines and applicable regulatory/accreditation requirements. Work includes: 1) supervising small staff of professional RN and social work staff who are directly responsible for coordinating the care and service of selected patients; 2) triaging care management emergencies and assigning work coverage; 3) analyzing and coordinating care provided by staff (including physicians and nurse practitioners), ensuring that adequate staff are available to meet patient needs; 4) monitoring documentation, quality, timelines and appropriateness of patient care (including care provided by physicians and nurse practitioners); and 5) providing staff education for new policies and processes. An incumbent will also perform direct patient care within scope of RN licensure as needed.

Work requires knowledge of the concepts, professional standards and methodologies applicable to the palliative care discipline, and of applicable regulatory requirements sufficient to ensure quality and compliant patient interactions and interventions care and exercise judgment/critical thinking in applying clinical guidelines and standards to determine the appropriate level of care. Also requires expertise in supervising and evaluating the work of subordinate clinical staff. Major decisions (including policy changes) are subject to the review and approval of higher-level management.

Qualifications

Job Requirements:

Graduation from an accredited school of nursing, BSN preferred.

Two years of related nursing work experience in an acute clinical care setting that would demonstrate attainment of the requisite job knowledge skills/abilities.

Work experience in one of the following clinical specialty areas is required: Oncology, Med/Surg, Nutrition Support, Pediatrics, IV Therapy, Gerontology, and Home Health.

Current licensure as an RN by the Washington State Board of Nursing.

Current Health Care Provider BLS Certification.

Current Department of Health Food Handler’s permit issued by the State of Washington.

Pay Range

$42.69 - $62.89 /hour
