Shift: 8am - 4:30pm

Job Summary:

Works with appropriate members of

the health care team to ensure coordination of patient care along the full care

continuum. Through their intervention, they will promote the efficient and

appropriate utilization of resources, identify and address care gaps, promote

health behavior changes for patients to better manage their illness and

recovery (if not chronic), analyze readmissions to the acute setting post

hospitalization and facilitate post-acute discharge transition arrangements. As

a result of their involvement there will be optimal clinical, financial,

functional and health status improvements in the target population.

Essential Duties:

  • Collaborates with physician andmultidisciplinary team in identifying appropriate patients for care managementreview, utilizing established care management criteria.

  • Assesses, plans and coordinates careon select patient population.

  • Seeks and evaluates availableoptions and services to meet an individual’s health needs and care teamrecommendations.

  • Promotes patient self-management andempowers patient/families to achieve optimal levels of wellness andindependence.

  • Collaborates with the healthcareteam members at hospitals, clinics, residential care facilities or othersettings, and health insurance payers to facilitate care across the continuumand optimize clinical and financial outcomes.

  • Serves as a liaison to providers,patients and families for coordination of services.

  • Provides resource information thatenhances patient and family competence and capacity to expedite decisionmaking.

  • Analyzes data or other materials foridentified care management metrics and quality improvement initiatives.

  • Effectively applies, interprets andcommunicates clinical guidelines, policies, procedures, regulations andstandards to key stakeholders.

  • Advocates on behalf of the patientand the organization to promote safety, quality and appropriate resourceutilization.

  • Actively engages with providers topromote quality and effective use or resources across the continuum of care.

  • Identifies opportunities to achieveclinical process excellence through a thorough analysis of available data andinvolvement of inter-disciplinary teams.

  • Informs and provides requirednotification to patient and/or personal representative regarding patient statusand determination

  • Leads readmission reduction effortsincluding risk satisfaction, pathway assignment, development and education ofpatients, physicians, and staff.

  • Responsible for overall care coordination,efficient resource utilization and discharge coordination of all patientsduring the hospital stay.

  • Uses discretion and independentjudgment in recommending appropriate clinical settings and levels of care forindividual patients by applying approved utilization management criteria andguidelines.

Qualifications

License/Certification:

  • Current RN license issued by theState of Oregon required.

Experience:

  • Three (3) years acute care nursingor long-term care experience required.

  • Experiencewith utilization management, discharge planning and/or case managementpreferred.

  • Must possess analytical and criticalthinking skills as well as the ability to prioritize (testing required).

  • Able to communicate effectively withpatients, families and all levels of health care providers.

  • Able to work independently withlittle or no supervision.

Education:

  • Graduate of an accredited school of nursing.

  • Bachelors of Nursing or related healthcare/socialservice field preferred.
