CHI Health has an immediate need to add an amazing Wellness Clinic RN (Nurse) to our Midlands Family Medicine Clinic located off of Hwy 370 & 84th St!

Hours: Monday-Friday 8a-5p, NO weekends & NO holidays

Responsibilities

*$5000 Sign On Bonus Offered*

This position will support our healthcare teams by ensuring they have everything they need in order to care for our patients. They also play a huge role in a patient’s experience and prep when they visit any of our clinics. We couldn’t do it without you! Expectations:

  • This position is for a Annual Wellness Visit RN. Wellness RN Work as part of a team to provide Annual Wellness visits to current Medicare patients past their 1st year of eligibility.

  • Help the patient and provider to create custom 5-10 year preventative screening plans for Medicare patients.

  • Provide education to help the patients understand how preventative care impacts their health.

  • Performs general nursing care to patients by following established clinic workflows.

  • May perform specific nursing care as it relates to specialty of the practice.

  • Educate patients and families about health status, health maintenance, and management of acute and chronic conditions.

  • Document patient assessment and intervention data using established medical record forms/automated systems and documentation practices.

  • Performs other duties within scope of role as assigned.

Qualifications

Minimum Qualifications:

  • Current Registered Nurse (RN) license that allows you to practice in our state

  • Graduate of an accredited nursing program

  • Basic Life Support (BLS) from the American Heart Association required

Pay Range

$26.38 - $38.26 /hour
