CHI Health would love to add a Clinic RN (Nurse) to our Internal Medicine team located at our CUMC-Bergan campus off of 77th & Mercy handling all the Wellness Nursing duties for our Medicare patients!
Hours: Monday-Friday between 7a-5p, no weekends, no holidays & no call!

Expectations:
This position is for a Annual Wellness Visit RN. Wellness RN Work as part of a team to provide Annual Wellness visits to current Medicare patients past their 1st year of eligibility.
Help the patient and provider to create custom 5-10 year preventative screening plans for Medicare patients.
Provide education to help the patients understand how preventative care impacts their health.
Performs general nursing care to patients by following established clinic workflows.
May perform specific nursing care as it relates to specialty of the practice.
Educate patients and families about health status, health maintenance, and management of acute and chronic conditions.
Document patient assessment and intervention data using established medical record forms/automated systems and documentation practices.
Performs other duties within scope of role as assigned
Qualifications
Minimum Qualifications:
Current Registered Nurse (RN) license that allows you to practice in our state
Graduate of an accredited nursing program
Basic Life Support (BLS) from the American Heart Association required
Pay Range
$26.38 - $38.26 /hour
