Job Summary / Job Purpose


medication history technician will provide assistance in the medication

reconciliation process by obtaining an accurate

medication history within 24 hours of admission when possible. Patients

shall be eligible and identified based on the following criteria: (1) anticipated admission from the Emergency

Department; (2) direct admit to the floor; and (3) transfer from another

facility. The certified pharmacy

technician will conduct interviews with

patients, family, or primary caregivers or other healthcare providers in order

to ascertain a complete and accurate medication list and have made available

for health care provider review via electronic medical record.

Essential Functions

  1. Demonstrate the AIDET (Acknowledge, Introduce, Duration, Explanation,

Thank You) process when interviewing patients.

  1. Utilizes available software to collect home medication list.

  2. In the event that the patient or patient’s representative is unable to

supply the medication list, the following may need to be contacted:

  1. Physician’s office

  2. Mail-order pharmacy

  3. Retail pharmacy

  4. Extended care facility

  5. Another acute care facility

  6. Obtains medication allergy details and immunization history

  7. Appropriately records the information obtained in the electronic medical

record .

  1. Communicates appropriate information as necessary to other healthcare


  1. Communicates effectively and effectively executes the medication

interview process with the patient and/or patient’s representative

  1. Shift rotations may be necessary in order to cover needs.

  2. Other duties as necessary to fulfill the needs of our patients including

but not limited too:


Proficiently preform all

functions of a Pharmacy Technician


Ensure the drug storage

locations are maintained in an appropriate manner


Restock ED MedSelect when

possible and on an as needed basis

d. Provide Constructive input

on how to improve Medication Reconciliation process

The Medication Reconciliation technicians will report

to the Emergency Medicine Clinical Pharmacists, the Pharmacy Clinical Manager and the Pharmacy

Residents as appropriate.


Job Requirements / Qualifications

Education / Accreditation / Licensure(required & preferred):


The High School

diploma, GED, or Associate degree in pharmacy technology


Kentucky Pharmacy

Technician License


National Pharmacy

Technician Certification


Basic typing and

computer skills


Basic mathematical



BLS certification

within 1 year of hire and continual maintenance

pExperience (required & preferred):

Minimium of 2 years of Pharmacy Technician experience required:


pharmacy experience preferred.
