Job Summary /Purpose:

The Licensed Practical Nurse (LPN), within their scope of practice and under direction and supervision of a Registered Nurse or Licensed Practitioner is responsible for contributing to the assessment, planning, implementation and evaluation of nursing care to a designated group of healthcare consumers across the continuum. Provides services which encompass the spiritual, physiological, social, and psychological aspects of care of the sick, prevention of disease, or conservation of health. The position contributes to the mission and core values of CHI Health.

Essential Key JobResponsibilities:

  • Under the direction and supervision of theRegistered Nurse or Licensed Practitioner follows the nursing process whichincludes: assess, plan, implement, and evaluate. The nursing process is anongoing process that can end at any stage.

  • Assessment – Demonstrates a systematic, dynamicway to collect and analyze the following healthcare consumer data including:psychological, physiological, sociocultural, spiritual, economic and lifestylefactors. Contributes to the RN initial assessment and may performre-assessment.

  • Plan – Sets measureable and achievable short andlong term goals according to standards of care based on the assessment by theRegistered Nurse in collaboration with the healthcare consumer and as part ofthe multidisciplinary team. Participates in the development and modification ofthe plan of care.

  • Implement – Implements and/or modifies the planof care using assessment data, clinical reasoning based upon standards of careand evidence based practice. This includes care coordination, education anddischarge planning. The nurse is accountable for the care directly, indirectlyor through delegation. Professional and legal standards for delegation must be followed.

  • Evaluate – Uses a systematic ongoing process toevaluate progress towards identified healthcare consumer goals. Revision ofplan of care when needed involving the healthcare consumer and themultidisciplinary team.

  • Documents all phases of the nursing process inthe healthcare consumer’s record.

  • Demonstrates personal and professionalresponsibility for the ongoing department, practice, and education. Maintains asafe work environment. Fosters collaborative environment in support of thenursing profession.

Minimum Qualifications

Required Education:

  • Graduate of a LPN nursing program

Required Licensure and Certifications:

  • Active LPN licensure in the state of Nebraska, Iowa, or nurse licensure compact.

  • BLS (Basic Life Support) from American Hearth Associate is required upon hire.

  • If hired in a NE facility, approved LPN-IV therapy course required upon hire.

Required Minimum Knowledge:

Knowledge of current nursing practice, critical thinking skills, professional verbal and written communication skills, technical/computer skills, attention to detail, customer service skills, adaptability, maintain confidentiality.
