The Admissions Coordinator identifies, communicates and coordinates on-going hospice home care services needed for the patient by reviewing the medical record, clinical assessment, communicating with the patient, significant others, health care worker(s), and insurance case manager. This activity may be in a health care facility or in the patient’s home. Initiates complex discussions about enrollment and end of life care and provides extensive information for patients and significant others.

  3. Follows the policies and procedures of the company.

  4. Other duties as required.

Admissions Coordinator

  1. Completes a comprehensive assessment of the patient’s holistic health status and needs to include clinical condition, level of care, support systems and resources, patient/caregiver knowledge, perception and adjustment towards disease process. Effectively communicates this information to the patient/significant others, attending physician and health care providers. Initiates complex discussions about enrollment and provides reassurance as appropriate to the needs of individual patients/significant others.

  2. Coordinates efforts with facility discharge planning staff and provides extensive discharge planning that involves selection of durable medical equipment for home use, transportation from discharging facility, setting up home health aides, and ensuring all medications and supplies have been ordered.

  3. Investigates/obtains insurance information on referrals. Works with case managers to negotiate coverage outside the insurance policy benefits.

  4. Initiates and discusses with patient/significant others the financial responsibilities of patient for medical insurance reimbursement, medication reimbursement, and levels of care.

  5. Demonstrates competency regarding hospice admission criteria and effectively communicates this information to patient, significant others, and health care team members.


  1. Graduate of an accredited school of nursing, registered nurse degree.

  2. Currently licensed to practice in the state serviced.

  3. Minimum of one year of registered nurse experience required, two years is preferred. One year of hospital RN experience is preferred.

  4. Must obtain and maintain current CPR certification; able to perform CPR.

  5. Compliant with appropriate state certification/registration and CHS requirements as necessary.
