Performs personal hygiene and grooming services.
Household care and provision of nutrition.
May perform duties under the direction of the RN, PT, OT, or ST.
High school graduate or equivalent.
Successful completion of a competency evaluation program.
Home health aide experience in the prior 24 months and/or passes the CHI Health at Home’s competency evaluation program as required by Medicare regulations.
Ability to understand the written care plans, execute instructions and document services delivered.
One year home health aide, patient tech or other related experience required.
Must obtain and maintain current CPR Certification (all locations) and any state required certifications.
