Clinical Staff Pharmacist

The Clinical Staff Pharmacist is a member of the multidisciplinary care team, providing clinical pharmacy services to patients including pharmacokinetic consultations, total parenteral nutrition consultations, anticoagulation management, antimicrobial stewardship, medication reconciliation, discharge counseling, formulary management, and compliance with clinical pathways and protocols. The Clinical Staff Pharmacist will work centrally and de-centrally to perform all functions of a clinical pharmacist including accurate dispensing, clinical rounding, patient involvement, and supervision of the technicians. The Clinical Staff Pharmacist will serve as a resource to other health care professionals through education and training programs. The Clinical Staff Pharmacist will apply clinical systems technology and distribution automation systems to provide effective, high quality, safe patient care.

Essential Job Duties:

  1. Coordinate and provide pharmaceutical care for patients on an assigned clinical unit and participate in patient care rounds with medical staff

  2. Pharmacokinetic evaluation and monitoring

  3. TPN evaluation and monitoring

  4. IV to PO conversions

  5. Anticoagulation consults and patient education

  6. Renal/hepatic dose adjustments

  7. Antimicrobial stewardship

  8. Therapeutic interchanges/Formulary Management

  9. Enter or validate new medication orders, maintaining a medication profile, using it to monitor patients’ drug therapy, drug interactions, duplication of orders, and contraindications

  10. Check and dispense medications prepared by pharmacy technicians

  11. Prepare medications for dispensing, including sterile IV admixture, and chemotherapy preparation

  12. Provide drug information pertaining to patient care

  13. Perform therapeutic drug monitoring to assess the pharmaceutical needs of patients

  14. Document recommendations and/or interventions

  15. Supervise pharmacy technicians

  16. Participate in professional activities within the department and in the community


Licensure or Certification:

  • Licensed as a registered pharmacist by the Tennessee State Board of Pharmacy.

  • Board certification desired


  • Minimum 3 years of clinical hospital experience or satisfactory completion of PGY-1 residency program required
