Medical Social Worker - Hospice of Nelson County

We are currently hiring a PRN Medical Social Worker for our Hospice of Nelson County location. Position coordinates care with other carevgivers and the attending physician to improve and/or maintain the social, emotional, functional and physical health status of the client.
Provides resources and education to patients and their caregivers
Participates in the coordination of services with other disciplines and community resources as needed
Evaluates and revises care plans/assessments based on changes in patients and/or their environments
Participates in regular case conferences and in-services or special case orientations as needed
Master’s degree from an accredited school approved by the Council on Social Work Education
Currently licensed (if required) to practice in the state serviced.
One year of social work experience in a health care setting required. Home care experience preferred
Must obtain and maintain current CPR certification
Proof of completion of Mandatory Reporter- Child and Dependent Adult Abuse training within three (3) months of hire where required.
